Most people with acne have spent years focused on the surface. Cleansers, acids, benzoyl peroxide, retinoids. Many of these work, to a point. But persistent acne — the kind that returns despite a rigorous topical routine — often has its root somewhere topicals cannot reach: inside the body, in the inflammatory pathways that drive breakouts at a cellular level. Addressing acne only from the outside is treating the symptom. Understanding it from the inside changes the approach entirely.

Why acne is an inflammatory condition

Acne is not simply a skin hygiene problem. It is a chronic inflammatory condition involving sebaceous glands, the skin microbiome, and the immune response. When the follicle becomes blocked and Cutibacterium acnes bacteria proliferate, the immune system triggers an inflammatory cascade. Cytokines — specifically IL-1, IL-6 and TNF-α — flood the area. The result is the red, swollen, painful lesion.

In inflammatory acne, this response is exaggerated or prolonged. Hormonal shifts (androgens increase sebum production), stress, and diet can all amplify the cycle. The lesions are not the starting point; they are the end point of a process that began well upstream.

This is why topical-only approaches have limits. A salicylic acid cleanser can remove excess sebum and surface bacteria. It cannot modulate the NF-κB pathway, the master regulator of the inflammatory response inside the cell.

What is MSM and how does it address inflammation?

MSM (methylsulfonylmethane) is an organic sulphur compound. Sulphur has been used in acne treatment for centuries — it is one of the oldest known skin interventions — but MSM is its most bioavailable form. It is found naturally in small quantities in foods but is most effective as a concentrated supplement.

At the cellular level, MSM inhibits NF-κB, the transcription factor that drives the production of pro-inflammatory cytokines including IL-1, IL-6 and TNF-α. By dampening this signalling pathway, MSM reduces the intensity of the inflammatory response before it becomes a visible lesion. Research has also demonstrated MSM’s effect on the STAT3 signalling pathway, which plays a role in immune cell proliferation and chronic inflammation.

MSM’s antibacterial mechanism

Beyond inflammation, MSM has a direct action against the bacteria responsible for acne. It works through iron-binding: MSM binds to free iron in the skin environment, making it unavailable to Cutibacterium acnes. Because C. acnes requires iron for growth and replication, this bacteriostatic mechanism creates a hostile environment for the bacteria without the harshness of antibiotic intervention.

This is a meaningful distinction. Antibiotics eliminate bacteria. MSM makes the environment inhospitable for them. For people with persistent or recurring acne, this is a more sustainable mechanism — particularly given the growing concern around antibiotic resistance in acne treatment.

MSM and sebum regulation

Excess sebum is central to acne formation. When the sebaceous glands overproduce oil, the follicle blocks more easily and the environment for C. acnes becomes more favourable. The sebaceous gland sits deep in the dermis. Oral delivery reaches it. Most topicals do not. MSM’s role in cellular health and its capacity to reduce systemic inflammation may similarly modulate the sebaceous environment over time, particularly with consistent supplementation.

Frequently asked questions

How long does MSM take to work on acne? Oral MSM typically requires eight to twelve weeks of consistent daily supplementation before significant changes in inflammatory acne are visible. Consistency matters more than quantity.

Can I use MSM if I am already on prescription acne medication? MSM is a naturally occurring compound and is generally well tolerated. However, if you are on isotretinoin, prescription antibiotics, or other systemic acne treatments, speak to your prescribing doctor before adding any new supplement.
